If you have been vaccinated but cannot provide documentation, you can obtain records from previous healthcare providers. Alternatively, you can undergo titer blood tests at our office to prove immunity. If titers show you are not immune, the required vaccines can be administered on-site.
USCIS requires proof of vaccination for the following: MMR (Measles, Mumps, Rubella), Hepatitis B, Varicella (Chickenpox), Influenza (seasonal), Tdap/Td (Tetanus, Diphtheria, Pertussis), Polio, Pneumococcal, and COVID-19. The specific requirements depend on your age and medical history.
Our Standard I-693 Package is $275.00 with 7-10 business day processing, and our Expedited I-693 Package is $350.00 with 3-4 business day processing. Both packages include the full medical examination, all required lab tests, and complete I-693 form preparation. Vaccines are additional if needed.
We accept both walk-ins and appointments. However, we strongly recommend booking an appointment to minimize wait times and ensure availability. Same-day and next-day appointments are available for your convenience.
A civil surgeon is a physician designated by USCIS (United States Citizenship and Immigration Services) to perform immigration medical examinations.
