1. Introduction
Welcome to Baron Medical Group. These Terms and Conditions ("Terms") govern your use of our medical services and website (www.baronmedicalgroup.com). By accessing our services or scheduling an appointment, you agree to be bound by these Terms. If you do not agree with any part of these Terms, please contact our office before proceeding.
2. Medical Services
Baron Medical Group provides a range of healthcare services including but not limited to: primary care, sports medicine, immigration medical examinations (Form I-693), regenerative medicine (PRP therapy, stem cell therapy), weight management (Semaglutide), and musculoskeletal care. All medical services are provided by licensed healthcare professionals under the supervision of Dr. Daesoon Leem.
The information provided on our website is for general informational purposes only and does not constitute medical advice. Always consult with a qualified healthcare provider for diagnosis and treatment.
3. Appointments & Cancellations
Appointments can be scheduled by calling our office at (213) 985-7777 or through our website. We require at least 24 hours advance notice for cancellations or rescheduling. Failure to provide adequate notice or repeated no-shows may result in a cancellation fee or the requirement of a deposit for future appointments.
- Cancellations must be made at least 24 hours before the scheduled appointment
- Late cancellations or no-shows may be subject to a fee
- Please arrive 15 minutes early for new patient appointments
- We will make every effort to accommodate scheduling requests
4. Payment Terms
Payment is due at the time of service unless prior arrangements have been made. Baron Medical Group accepts the following payment methods:
- Cash
- Zelle
- Venmo
- Credit/Debit Card (Visa, MasterCard, American Express)
Pricing for services will be communicated prior to treatment. You are responsible for any applicable co-pays, deductibles, or out-of-pocket expenses. If a payment dispute arises, please contact our billing department promptly.
5. Medical Records
Baron Medical Group maintains medical records in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable California state laws. Medical records are retained for a minimum of seven (7) years for adult patients and until the age of majority plus seven (7) years for minor patients, as required by law.
You may request copies of your medical records by contacting our office. A reasonable fee may be charged for record duplication and processing. Records requests are typically fulfilled within 15 business days.
6. Limitation of Liability
Baron Medical Group and its healthcare providers strive to deliver the highest standard of care. However, no medical treatment can guarantee specific outcomes. By using our services, you acknowledge that:
- Medical treatments carry inherent risks that will be discussed with you prior to any procedure
- Results may vary from patient to patient
- Informed consent will be obtained before any treatment or procedure
- Our website content does not create a physician-patient relationship
Baron Medical Group shall not be held liable for any indirect, incidental, or consequential damages arising from the use of our website or services, except as required by law.
7. USCIS / Immigration Medical Exam Terms
Baron Medical Group is authorized by USCIS (U.S. Citizenship and Immigration Services) to perform immigration medical examinations and complete Form I-693 (Report of Immigration Medical Examination and Vaccination Record). Please note the following:
- The I-693 form is valid for two (2) years from the date the civil surgeon signs the form
- All examinations are conducted in strict compliance with USCIS Technical Instructions
- Required vaccinations must be administered or documented proof of prior vaccination must be provided
- A valid government-issued photo ID is required at the time of examination
- Baron Medical Group is not responsible for USCIS processing decisions or immigration outcomes
8. Website Use
All content on the Baron Medical Group website, including text, images, logos, and design elements, is the property of Baron Medical Group and is protected by applicable intellectual property laws. You may not reproduce, distribute, or modify any content without prior written consent.
9. Changes to These Terms
Baron Medical Group reserves the right to update or modify these Terms at any time. Changes will be posted on this page with an updated "Last Updated" date. Continued use of our services after changes are posted constitutes acceptance of the revised Terms.
